You may request an appointment for a cosmetic dermatology procedure such as Botox, Filler, Laser Tattoo Removal, Laser Brown Spot Removal, Peel, Latisse, or lunchtime peel or you may request an appointment for skin cancer or Mohs surgical consultation, lastly, you may request an appointment for a general dermatology problem.
Upon confirmation of your appointment with our staff, you may download the New Patient Forms below. Complete them prior to coming in for your appointment. This will save waiting time for you in our office.

Dermatology Insurance Information
Unfortunately, at this time we accept no HMO’s.
We welcome all patients but the only insurance we accept is Medicare.
We will gladly see all other patients, regardless of their insurance, but payment is due at the time the service is rendered.
We will provide you with a receipt on the day services were provided so that you may submit to your private insurer for reimbursement, where applicable. Our fees may be higher than your insurance company reimburses you. Our office staff is happy to provide you with our fees prior to your appointment.
If we are not on your plan, payment is due at the time of the visit, and we accept all major credit cards, cash, and personal check. We will provide you with a superbill that you can submit to your insurance for out of network reimbursement as permitted by your plan. Each insurance plan is different. For all cosmetic and medically unnecessary procedures and services payment is due at time service is rendered and is not covered by insurance.
